
Hypertension is one of the most common conditions in CKM Syndrome, and one of the hardest to judge from a single visit. A reading taken in the office can be high from a rushed morning or low on a good day. Either way, it is one number standing in for weeks of life.
What the prescriber needs
When a clinician starts or changes a blood pressure medication, the question is simple: is it working, and is the patient tolerating it? Answering it well takes readings over time, plus context: missed doses, dizziness, headaches, or a change in salt or activity.
Home readings, collected simply
Patients who check their blood pressure at home can report it in a short daily or weekly check-in through the PDS™ Patient Data System. The care team chooses the questions, for example:
- What was your blood pressure this morning? (top and bottom numbers)
- Did you take your blood pressure medication today?
- Any dizziness or headaches today?
Readings become a trend the prescriber can review before the next visit. Readings outside the range the team sets become an alert, so EBM can follow up and escalate to the physician when needed.
Protecting the heart and kidneys
Blood pressure that stays high strains the heart and the kidneys. That is why hypertension sits at the center of the CKM cluster with heart disease, chronic kidney disease and Type 2 diabetes. Keeping it in range is one of the most direct ways to protect the rest of the patient.
