Chronic kidney disease (CKD) is the “K” in CKM Syndrome, the Cardiac, Kidney and Metabolic cluster. It is often quiet in its early stages, and much of what affects it, including blood pressure, blood sugar, fluid and medications, is managed at home between appointments.
Why the rest of the cluster matters
Kidney health is tightly linked to the other CKM conditions. Long-standing hypertension and Type 2 diabetes are leading drivers of kidney damage, and kidney disease in turn raises the strain on the heart. Supporting a CKD patient means following the whole picture, not one lab value.
What a kidney-focused check-in tracks
With the PDS™ Patient Data System, the nephrology or primary care team chooses a few short questions, such as:
- Did you take all of your medications today?
- What was your blood pressure this morning?
- Any new swelling in your legs, feet or face?
- How is your energy today?
Answers become trends. When something crosses a threshold the team set, CareConnect™ raises an alert so EBM can reach the patient and escalate to the physician.
Every specialist, the same picture
A CKD patient may see a nephrologist, a cardiologist, an endocrinologist and a primary care physician. CareConnect™ connects them and the patient in one channel, which is the idea behind the Medical Neighborhood. A medication change one specialist makes is visible to the others, along with how the patient responded.
EBM does not add work to the clinic. Our team handles the between-visit workload, escalates what needs attention and documents everything, while physicians stay in control of every clinical decision. There are no CPT codes required, no payer qualification, and no hiring, equipment purchase or IT build-out. Anyone in the chronic population can participate, regardless of age, diagnosis or insurance type.
